Best to have them on separate sends. If you put them all on one track, you won't be able to use the long reverb on a snare, and the short reverb on a synth, for instance. Having each on a separate send will allow you to determine which instruments get which reverb, and how much is added.

If you maybe have a dusty old controller of some sort plugged in, that can be the culprit. Dust in the encoders will cause some ghost MIDI activity. Just spray it out with some canned air. But if it still happens and you don't have any controllers plugged in, I have no idea.
