Have you tried disabling any extraneous MIDI drivers that may be present?
I'm sure you've tried everything though at this point...

I'm having the same exact issue - massive CPU spikes registering around 300% on the CPU load, and near 100% on my Mac OS 10.6.3 Activity monitor. I created a patcher that generates a flurry of MIDI notes (long story), and my intention is to feed them into Live in order to control keymapped parameters, or instruments on other tracks... hmm.. I've had a similar experience with Sibelius... it seemed that my interface sample rate wasn't jiving with my sound card's sample rate. They were both the same, but who knows.. having both programs open at the same time seemed to be the culprit. See if you have anything else running; it sounds ...
