If you're using windows then try running Live as administrator and then rescanning your VST folder again.
Some plugins will require you to run Live as administrator all the time.

Sorry, cant help you with who originally came up with the Formant filter, but it certainly wasn't Ableton.
Lots of other instruments and plugins have a formant filter. Ableton just happen to have taken the shape of the Formant filters and provided them as presets for EQ8.

Dont use the crossfader. Mix with the channel faders instead.
Even on a hardware DJ mixer, the crossfader is only really used for scratching (if you're into that sort of thing).
