Routing and settings for recording midi track to audio track

i get so confused with the monitoring / audio to / audio from / etc etc

this is the scenario.

midi track (let's call this A) that i want to record with "inserts" post "sends" into a new audio track (lets's call this B)

what do i put in the audio to, audio from category on each track?
what do i put in the monitor category on each track? (in? auto? off?)


For some reason my levels in the record enabled audio track are waaaaay too loud every time. I pretty sure i'm not double monitoring..... also, the fader matches the one from the other channel in volume so it can't be that.....

anyway, help greatly appreciated
