msb/lsb/program numbers still start at 1 instead of 0

i just spend a whole afternoon figuring this out
why isn't this changed or at least better documented?

as far as i can tell, it is only mentioned in some obscure Live 4 Beta testing topic. according to one of your employees (eduardo?) it would be changed to comply with midi standards.
since this hasn't happended yet, at least write something in the manual about it.

some extra info:

- if the needed msb/lsb/program value is 0, either 1 or 'none' works

- if the needed value is above 0, + 1 needs to be added

examples:

* 127-0-80 on your synth can be selected in Live by either 128-none-81 or 128-1-81

* 3-112-13 would be 4-113-14
