Ubermap configs - share your VST/AU device mappings

A great idea suggested by peterswimm was to create a new thread for Ubermap device mappings. I'd like to find a better way to share them but this will certainly do for now!
What I will do is maintain an index in the post below to all the mappings that are posted, so it's easy to see what is here. Please try and keep this thread on topic.
Thread rules:

For those of you who don't know, Ubermap lets you map custom layouts on Push for VST and AU plugins. There's more info in the thread at
