I think he means that the Bitwig thread starts when hype in the forum begins to divide out of control at an exponential rate. If the hype stayed in one place, then the Bitwig threads would usually be relatively easy to treat by surgery. But Bitwig threads can grow and spread around the forum, making treatment more difficult.
