In Ableton, how can I distort the pitch and the tempo of a sample, like in the video below?
(Fast forward to 4:05 in the video)

I can think of a couple of ways.
You can set the warp mode to "Re-Pitch" and automate the track tempo. If you want the pure audio file from that you can export it.
Or you can load the sample into a sampler and pitch bend it.

Okay. I'm still not sure I'm doing it correctly. In re-pitch mode, anytime I change the pitch with that :2/*2 thing, it changes the pitch of my entire clip, as opposed to just one segment.
How do I go about creating this effect in (changing the pitch and tempo) in Sampler?

In re-pitch mode, if you want to change the pitch on a part, you have to make sure to add another warp marker that tells Ableton Live, that there will be a tempo change.
What might work better instead though, is having the whole clip at one consistent tempo, and then changing the Master Tempo of Ableton Live to meet your needs
