Program change and control change

Hi, i'm new to all the music production environment but i've been learning a lot from the Ableton lessons and pther stuff i've been reading/watching. But there's something i still can't grasp.
I have an Akai mpk mini MIDI controller and there are 2 functions on it: CC and Prog change. What exactly are these for and how would i use them in Ableton?
I know it's a very basic question but i've got to start somewhere.

Use CC values to contoll the virtual knobs/sliders in ableton via your mpk knobs/sliders. Use program change on your mpk pads to change presets in your vsts.

So from MPK or otherwise, is there STILL (I've been out of the loop for a couple of years) no way to change Live (non-plugin) synth patches from program control messages??

I'm not seeing any "Ableton patch bank manager"-type function...
