From the looks of it your ports are still there but your configuration has been wiped; there is a difference ;-)
Anyway; what I'd do is to shut down Live, pull out all your midi stuff and start with only having the APC40 hooked up. Now start Live up again and check the midi preference tab.

The downside: You can't control the tempo by using the designated tempo-encoder on the Remote SL, but not having to use the Automap server is more important to me. That is strange that its showing outputs for this device when you said there are no real hardware outputs on the usb mic. Maybe the driver is misleading ableton to think it has outputs, regardless you want to use the stereo out on your mac, correct? If so you will have to aggregate(join) your "usb ...
