(That explains why I didn't have any fluctuations but just a bit of delay.)
So I can say: recording the MD works perfectly without any fluctuations or delay when I turn off Live's monitoring for that channel. At least for me

this is for pc users as midi-yoke does not work on mac.
download midi-ox and midi-yoke and install.
send your sync signal from ableton to midi-yoke 1
in midi-ox send midi signal from midi-yoke 1 to
the midi that is going to your outbound gear.
somehow it seems like midi-ox takes the time
code and cleans it up, somehow reducing jitter.
i have to use delay compensation on my individual midi tracks in
ableton to get them right, but now that is syncing perfectly also.
hopefully this isn't some bizarre thing that works only on my machine and will help somebody else!
oh and in case you were wondering i was having the exact same jitter problems
with two different soundcard/midi interfaces, so the problem was not in those things.
