How to automate hardware preset changes by sending MIDI CC

I couldn't find a good answer on the forum and Ableton hasn't gotten back to me so here I go...

I want to use Ableton to send MIDI CC data to my rack gear as my live set progresses for hands free operation from my rack gear. I run backing tracks and want to have Ableton automatically send the MIDI CC data to my gear as the tracks are playing.

Any help would be greatly appreciated.

(if you're familiar with ENTTEC's DMXis plugin, its sorta the idea I'm going for. Only instead of sending DMX data I want midi cc data to be sent).

OK... So you set up a MIDI track in Arrange mode. and draw midi notes into the Piano Roll. connect your midi interface and select that and link it to your hardware. make sure you are on the correct channels. Once you press play and the track plays past your midi notes you should see your hardware receive MIDI IN.

NOW my question is... How do you know what MIDI notes are sending what value. as in 0-127 to the device?

Any help would be awesome!
