The MME port type is located where? The only reference (I could find) to MME is in the audio selection page, not in the MIDI page, unless I am missing something. I switched the audio settings to MME but I had no sound (I am using an M-Audio 2496 card) and current audio settings are with ASIO.

Is there additional settings within the MIDI I/O page for MME selection?

leave the audio settings 'ASIO'.
You can choose the port type for all MIDI ports in the Midi/Sync preferences. Click on the little triangle next to the port names to unflip further options for the ports - you´ll find the 'port type' settings there.
