Have seen that many folks seems to have problems getting the knobs on the Akai MPK Mini to map in Live, myself included.
Here's what worked for me in 8.2.2, pretty easy. Go to Options/Preferences/Midi Sync and look for the Input: MPK Mini in the "Midi Ports" area. Make sure that the box for "Remote" is checked. You should be good to go at that point.
I also found that I needed to have my MPK Mini plugged into the USB port of my laptop BEFORE I start Live, otherwise it's not seen in Live if I plug it in after starting.
Also, I could not find any "return to default" for the MPK. So, I created a "default" setup file that I use with the MPK Editor to reset the MPK if needed. And I've actually noticed the same issue with all my MIDI controllers- if they aren't plugged in before I start Live they aren't recognized- it also means that if something gets unplugged by accident while I'm using Live I have to restart it. I wish Live had some sort of "Rescan" feature for MIDI devices so to allow more hot-swapping
Anyhow, glad you got the MPK to work- it's sort of strange that people are having so many issues with it

as for windows, well, windows locks up drivers to applications so if you accidentally fuck up then all you can do is to shutdown your application, replug your controller and reboot your application.
unless ableton can figure out a way to bypass that, a rescan feature will never happen, and i highly doubt that they can.
in short: if you're going out live on a windows os, make sure to fucking glue your usb ports in place, because if someone accidentally unplug it you're screwed.
