Chances are that it's what your instrument is playing, i.e. the MIDI clip. Double-click on the clip and check the Program settings: Bank/Sub-bank/Program. Make sure they are all set to "None".

This was down to my MIDI clip getting imported with a default of 1 set in the Bank | Sub Bank | Program settings. I switched them all to "None" and, bingo, no more unwanted patch changes!
